Award Finalist Submission Form:

Submit your completed submission form by 5pm, Monday, November 15, 2004.

Please submit a 150-word description outlining the merits of your candidate person, team or project for consideration by the event’s selection committee. In addition to being used for selection purposes, this description, and accompanying artwork, will become the basis for your display in the Hall of Nominees. The Hall of Nominees is a series of displays on view during the Champagne Reception just prior to the awards event itself. This reception gives attendees the opportunity to tour the various nominees, and learn more about the people and projects in a relaxed and festive atmosphere. The artwork accompanying your description should be camera ready and not include company logos or promotional information. Each photo should be accompanied by an appropriate caption. Please note that the photo, as well as information in this caption, are for descriptive purposes only and will not be considered by the committee in the selection process.

To be considered for the final round, nominees must submit additional details describing their person or project. This information should be accompanied by appropriate artwork sent via e-mail to lcarls@oilonline.com and must be received no later than 5pm Nov. 15, 2004. CD or USB drive submissions can be mailed to Lisa Carls, OECE Conferences & Events Manager, 1635 W. Alabama, Houston, TX 77006. Mailed submissions must be received no later than 5pm, Nov. 15, 2004 to be considered.

Photo Specs: all photos or photo collages should fit within a 18" x 24" horizontal space.
